In PvP players do not play simultaneously.
They change active player when they are finished with their play.
It looks like this:
Combat starts when the attacker moves on the hex with another player.
At this point the attacker may play anything he wants, but when he asks "Do you attend partially or fully?" he is changing the active player and now defender announces if he fights fully or partially. Ater that he starts his Ranged phase. He can do whatever he wants during this phase - use dice (if he can) play cards and when he is finished the attacker he is back in action. He can defend if there is any attack against him.
If there is no attack he can shoot at his target or skip this phase and starts melee attack. Combat finishes when one player is pushed back or both passes.
So who gets the desired die depends who will take it in the right moment during his actions...
You play your cards during your phase. When a card allow you to do something you can do it. I'm not sure which spell you have in mind.

Thanks, your explanation helps (assuming it is correct ;)). According to what you've written, the answers to my questions seem to be:
1. If one player has expose and one has tremor, it is the defender who can play first (and therefore gets the die).
2. The attacker should be able to use tremor before assigning damage, as it is in his part of the turn.
3. The blocker cannot do anything after playing his blocks, so no banners/crystals
